Vuelopedia is a work in progress: it currently holds about 30% of the information we expect to have by the third quarter of 2027. Although we use only official, highly reliable sources, some aircraft data and history may still contain errors or inaccuracies. We work hard to clear every anomaly from the records using specialized verification tools.

What happened
On 23 October 2011, at about 16:00 local time, a Boeing 747-451 (N661US) with Pratt & Whitney PW4056 engines suffered a low pressure turbine failure of the left inboard (number 2) engine after taking off from Detroit (DTW). The crew noticed a heavy compressor stall and a dull explosion, followed by the number 2 engine spooling down and the fire warning; they returned and landed on three engines without incident and with no injuries. There was minor impact damage to the left wing, flaps, ailerons and stabiliser, and an under-cowl fire with punctures in the turbine case. Disassembly revealed that a segment of the low pressure turbine stage 3 duct, of an older design with a riveted seal prone to failure, came loose and destroyed the blades; its fragments punctured an oil line, whose spray ignited on contact with the case. The engine did not meet the containment standards then in force. Source: the NTSB (United States).
