Vuelopedia is a work in progress: it currently holds about 30% of the information we expect to have by the third quarter of 2027. Although we use only official, highly reliable sources, some aircraft data and history may still contain errors or inaccuracies. We work hard to clear every anomaly from the records using specialized verification tools.

Accident at Miami International Airport
On November 20, 2000, the Airbus A300B4-605R aircraft (registration N14056), operated by American Airlines, was involved in a fatal accident at Miami International Airport (United States), leaving 1 dead. Source: American Airlines.
What happened
Shortly after taking off from Miami, this Airbus A300 suffered a pressurisation system failure. The crew switched the system to manual control and returned to the airport. During the return several lavatory smoke warnings activated, although no fire was found. The chain of failures was completed on the ground. The pilots did not complete the manual pressurisation or emergency landing checklists, both of which required the aircraft to be depressurised before landing. Stopped on a taxiway, the captain saw the aft cargo compartment fire warning illuminate and ordered an evacuation. The crew tried to open the doors and they would not move: the aircraft was still pressurised. The lead flight attendant persisted with the forward left door, which opened explosively and threw her to the ground, causing fatal injuries. The investigation noted that the emergency evacuation checklist did not require depressurisation to be verified before ordering the exit. It is a textbook case of how a minor technical failure, combined with incomplete checklists, can end in a death.

What happened
On 11 September 2001, after the attacks with hijacked aircraft on the Twin Towers in New York and the Pentagon, the FAA ordered — for the first time in history — the complete closure of United States airspace (the SCATANA plan): all civil aviation was told to land immediately, and by midday the American sky was clear. Around 240 international flights bound for the United States were diverted to Canada — Operation Yellow Ribbon, with more than 30,000 passengers. Airspace reopened gradually from 13 September, by then under new security measures. Source: the FAA (United States).
