Vuelopedia is a work in progress: it currently holds about 30% of the information we expect to have by the third quarter of 2027. Although we use only official, highly reliable sources, some aircraft data and history may still contain errors or inaccuracies. We work hard to clear every anomaly from the records using specialized verification tools.

Accident at Phoenix Sky Harbor International Airport
On August 28, 2002, the Airbus A320-231 aircraft (registration N635AW), operated by America West Airlines, was involved in an accident at Phoenix Sky Harbor International Airport (United States) due to a runway excursion caused by asymmetric thrust on landing, leaving 10 injured. Source: the NTSB (United States).
What happened
Arriving from Houston, the A320 landed on runway 08 at Phoenix with the No. 1 engine thrust reverser deactivated and locked out by maintenance since 20 August. During the landing roll, while moving the No. 1 thrust lever out of reverse, the captain inadvertently advanced it to the take-off/go-around (TOGA) position while the No. 2 lever remained in full reverse; the aircraft yawed right, left the runway and the nose gear collapsed in the infield. One occupant was seriously injured and nine received minor injuries. The NTSB determined the probable cause to be the captain's failure to maintain directional control and his inadvertent application of asymmetric thrust, with crew coordination as a factor.
